　　（一）术前　　近年来，经过多中心、大样本的前瞻性观察，围手术期营养支持对降低手术死亡率和并发症发生率的效果已为临床医师所共识，并认为术前纠正营养不良的效果优于术后营养支持。研究认为，术前营养不良可引起患者出现低蛋白血症，体液和细胞免疫功能障碍，组织修复能力下降，延缓术后伤口愈合，增加宿主对感染的易感性及并发症发生率，并延长住院时间，增加住院费用。　　术前并非所有的患者都需要营养支持，只有明显营养不良的病人需要额外补充。外科病人术前的营养不良可能由胃肠功能障碍、食欲不振、腹泻、消化道部分梗阻等或营养素丢失过多造成。因此，术前就存在营养不良的病人，首先应该查明发生营养不良的原因，尽力纠正原发病；其次，根据营养状况评定，明确是何种营养物质缺乏，根据具体情况进行相应补充。术前营养支持的途径可根据病人的情况而定，肠内途径为首选，不足者辅以肠外途径。一般来讲，术前营养支持的时间为7～14天，如果病人情况差，可予适当延长。对于术前已不能正常进食者，更应争取足够时间获得补充。　　（二）术后　　1.补充病患生命所需、调节电解质平衡等在本章的第二节，已经对外科手术后机体的代谢改变进行了介绍。众所周知，合理的营养支持对于改善术后应激状态下病人的营养不良状况，减少蛋白质丢失，增强免疫力，促进修复和维持器官功能具有重要作用。特别是对某些重症或经历大手术的病人，营养支持的效果确切而显著，如肠外瘘、短肠综合征、重症急性胰腺炎病人。　　但并不是每一个术后病人都需要进行营养支持，一周内能恢复60%饮食的病人或是没有营养不良的病人都不需要营养支持，仅给予水、电解质补充和输注150～200g葡萄糖即可。即使是合并慢性呼吸、肾或肝功能障碍或是老年病人，除非有重度营养不良，也不需要术后给予营养支持。而且，营养支持对于已有显著营养不良的病人实际上是一场“代谢应激”，营养不良越严重，支持强度越大，应激也越显著。因此，营养的补充不宜过少或过多，过少不足以满足机体需要，过多则将加重器官负担而产生副作用。　　对于那些术后发生了并发症，需要营养支持的病人。可概括起来按术后早期、并发症出现期和康复期三个阶段来讨论营养支持。术后早期营养支持的作用在于供给机体组织愈合、器官功能恢复、免疫调控所需的能量与营养底物，在术后24～48小时内，内稳态得到稳定后即可进行，仍然是胃肠道营养为首选。由鼻肠管或术中预置的胃造口管、空肠造口管给予。如胃肠道不宜灌注时，可采用肠外途径。　　……
